Taxes & ACA 1095-C Forms
For employees eligible for medical insurance coverage offered by St Johns County, look for your 1095-C form to arrive in the mail starting March 2. Although the 1095 form is not needed to complete your taxes for 2019, employers and insurance providers are still required to furnish these forms to their employees. Please check with your tax advisor if you have any questions related to your 2019 tax filing. More Information about 1095-C Forms.
Tuition Reimbursement for FY 2020
Tuition Reimbursement Applications can be submitted via fax 904.209.2414, interoffice mail or email your Benefits Team. Please contact your Benefits Team at 904.209.0635, option 5 or visit the Training & Development page for more information on the County’s tuition reimbursement program including eligibility and the application process.
Conduct a Paycheck Check Up
It is always a good idea to perform a “paycheck checkup” for income tax. The IRS provides a Withholding Calculator to help you determine if you are withholding the correct amount. If you would like to make a change to your tax withholding; fill out a new 2020 W-4 form and return to Payroll via fax 819-3697.
CVS Caremark ® and Diabetic Supplies
As a reminder, diabetic supplies are only covered by the CVS Caremark plan, and not by Florida Blue medical. To find out if you or a covered family member are eligible for a no-cost Diabetes Accu-Chek Blood Glucose Meter, call the CVS Caremark Member Services Diabetic Meter Team at 1-800-588-4456. Have your prescription ID number and your doctor's name and phone number ready when you call. CVS CM resources to help you manage diabetes.
