Effective January 15, 2022, CVS Caremark will cover over-the-counter (OTC) COVID-19 tests without a prescription.
You can access this benefit in one of two ways:
Present your prescription ID card at any in network pharmacy that elects to offer COVID-19 tests through this program. Through this option, members can obtain an OTC COVID-19 test with no upfront out of pocket cost.
Purchase OTC COVID-19 tests from any pharmacy or merchant of your choice and save your receipt. Submit for reimbursement of Over-the-Counter (OTC) COVID-19 tests through the CVS Caremark website or submit the COVID-19 Test Reimbursement Claim Form.

Who is eligible for an Over-the-Counter (OTC) COVID-19 test reimbursement claim?
You are eligible if you have private or employer-sponsored health insurance.
You are not eligible if you have Medicare, Medicare Supplement, Medicaid, or voluntary insurance.
This new coverage reimbursement policy applies to over-the-counter (OTC) COVID-19 tests bought without a prescription.
How many OTC COVID-19 tests are covered?
Your plan will cover up to eight OTC COVID-19 tests without a prescription per 30-day period for each person covered under your plan.
If you buy a multi-pack of tests, each test in the package counts as a single test. So a four-pack counts against the limit as four tests.
You do not need an order from your doctor to get reimbursement for your test. However, it’s important to know that there is no quantity limit on tests ordered by your doctor or other health care provider.
Tests must be used to see if you or a covered family member has a COVID-19 infection. You won’t be reimbursed for tests used for work, school or other requirements.
